Certificate of Transfer Credentials (Honorable Dismissal) from the last school attended
Informative Copy of Transcript of Records (with school seal and date of graduation)
2×2 colored picture (any background)
PSA Birth Certificate (Marriage Certificate for married female students)
Certificate of Employment (at least 1 year of relevant work experience)

CONTINUING STUDENTS/USJ-R GRADUATES

(USJ-R AS THE LAST SCHOOL ATTENDED)
